Who Are We?
Patient First Hospice & Palliative Care (PFHPC) is a Minority Women-owned hospice agency based in Houston, dedicated to providing compassionate, culturally responsive end-of-life care. Founded and led by a Registered Nurse with over 20 years of hospice and healthcare experience, PFHPC is committed to honoring each patient’s dignity, comfort, and unique needs.
We deliver comprehensive hospice care for individuals and families facing terminal illness, focusing on comfort rather than curative treatment. Our interdisciplinary team provides pain and symptom management, emotional and spiritual support, and a variety of specialty programs—including our Dementia Care Program, Respiratory Support Program, Homemaker Support Program, Weekend CNA Support, Remembrance Program, and extensive Bereavement Services.
PFHPC provides care wherever the patient resides—whether in their home, with family, or within a facility—ensuring compassionate support throughout their journey.
About Hospice
Hospice care is a special kind of care focused on quality of life for people experiencing an advanced, life-limiting illness. It provides comprehensive medical, emotional, spiritual, and practical support to patients and their families. Hospice is not about giving up; it’s about ensuring comfort, dignity, and peace.
Office Hours and On-Call Availability
Our office is open Monday through Friday from 8:00 AM to 5:00 PM. However, our hospice care team is available 24 hours a day, 7 days a week, including weekends and holidays. If you have an urgent concern or need assistance at any time, please call our main line at 832-916-3552.
Your Core Hospice Team
Hospice Physician: Oversees and coordinates your medical care.
Registered Nurse (RN): Manages symptoms and medications, and provides patient and family education.
Certified Nursing Assistant (CNA): Helps with personal care, including bathing and grooming.
Social Worker: Offers emotional support, assists with financial and legal planning, and connects families with community resources.
Spiritual Care Coordinator / Chaplain: Provides spiritual counseling, regardless of religious affiliation.
Volunteer: Offers companionship, assistance with errands, and respite for caregivers.
Bereavement Coordinator: Provides grief support to family members for up to 13 months after a patient’s passing.
When to Call Your Hospice Team
Please call us immediately if you notice any of the following changes:
• Pain not relieved by medication
• Fever over 100°F
• Rapid or irregular heartbeat
• Increased confusion or excessive sleepiness
• Skin changes such as redness or sores
• Swelling in legs or arms
• No urine output for 24 hours
• No bowel movement for 3 days or no colostomy output for 48 hours
• Persistent nausea, vomiting, or diarrhea
• Difficulty swallowing
• Sudden shortness of breath
• Any fall or physical injury
• Suspected or confirmed COVID-19 exposure or symptoms
