Patient First Hospice & Palliative Care (PFHPC) is a Houston-based, Minority Women-owned hospice agency. We pride ourselves on delivering compassionate end-of-life and palliative care. The owner, Erica Jordan, RN is a registered nurse with 20+ years in hospice and healthcare experience. PFHPC provides comprehensive care and support for individuals and their families with terminal illnesses, focusing on comfort and dignity rather than curative treatments. We offer a range of services and specialty programs, including pain and symptom management, emotional and spiritual support, dementia program, respiratory program, music therapy, pet therapy and bereavement services for families, all within an interdisciplinary team approach. We provide care in the patient's home, however care can be wherever the patient is located.
Hospice care is a special kind of care focused on quality of life for people experiencing an advanced, life-limiting illness. It provides comprehensive medical, emotional, spiritual, and practical support to patients and their families. Hospice is not about giving up; it’s about ensuring comfort, dignity, and peace.
Our office is open Monday through Friday from 9:00 AM to 5:00 PM. However, our hospice care team is available 24 hours a day, 7 days a week, including weekends and holidays. If you have an urgent concern or need assistance at any time, please call our main line at 832-916-3552.

Yes. We collaborate with your primary care physician, or you can choose our hospice physician. However, you would no longer be able to see specialist related to your terminal diagnosis. The Hospice Medical Director will manage your terminal condition.
Medicare, Medicaid, and most private insurance plans fully cover hospice services.
Medical care, medications for symptom control, equipment (hospital beds, oxygen), personal care assistance, emotional and spiritual support, and grief counseling.
Yes, you may revoke hospice care at any time and return to curative treatment if you choose. However, If you have a change in condition you can elect to receive hospice later.
You can stay on hospice services as long as they meet the eligibility criteria:
• The patient must have a terminal illness with a life expectancy of six months or less if the disease follows its normal course.
• This prognosis must be certified by a physician.
• Hospice services are provided in 90-day and 60-day benefit periods (two 90-day periods, then unlimited 60-day periods).
• At the start of each new benefit period, a hospice physician must recertify that the patient still qualifies for hospice care.
So, there is no fixed limit to how long a patient can remain on hospice, as long as they continue to meet the medical criteria for eligibility.
In Texas, a physician’s order is not required to evaluate a patient for hospice care. Hospice agencies can conduct an initial assessment to determine if a patient may be eligible for hospice services. However, before hospice care can officially begin, a physician must certify that the patient has a terminal illness with a life expectancy of six months or less if the illness runs its normal course.
